About Tranquil Life Psychiatric

Our Mission

At Tranquil Life Psychiatric, our mission is to provide compassionate and effective psychiatric care to our patients. We believe in a holistic approach to mental health and work closely with our patients to develop a personalized treatment plan that addresses their unique needs.


Our practice is family focused and treats children, adolescents and adults  (ages 8 and above).

Our Provider

  

Joseph Holshoe, BS, BSN, MA, MSN, PMHNP-BC, is a board-certified family psychiatric nurse practitioner with a combined 30 years’ experience as a psychiatric nurse and nurse practitioner. He has extensive experience in the treatment of anxiety, depression, PTSD, ADHD, substance abuse, and sleep disorders. He is a nationally recognized speaker on sleep, military psychiatry, PTSD, and mood disorders and is the author of the chapter on “Antidepressants and Sleep” in the Encyclopedia of Sleep.  He is also the co-chair of the American Psychiatric Nursing Association's [APNA] Clinical Psychopharmacology Institute

Required Documentation

All required documentation must be completed 24 hours prior to your appointment

Required forms can be found on the Client Portal via the "Book An Appointment" toggle at the top of this page.


Failure to complete required forms prior to your appointment may result in your appointment being rescheduled or cancelled.

Payment

Payment is due on the day of your appointment. Credit card information is managed on the secure Client Portal and a valid credit card is required to secure your appointment.

Policies

Cancellation & Missed Appointments

24 hour notice is required in order to cancel an appointment without charge or fee. Charges for no-shows and appointments not cancelled 24 hours in advance are $175 for new patient assessments and $100 for follow up appointments.


Missed appointments/no-show charges are NOT covered by insurance.  The patient is responsible for missed/no-show appointment charges.


Co-Pay & Insurance

Co-pays are required on the day of your appointment.


Please confirm authorization for care with your insurance provider prior to your initial appointment.


Please provide your insurance information and insurance card at each visit to confirm eligibility of benefits.


Fees not covered by insurance are the responsibility of the patient and/or patient's responsible family member.


Medication Refills

Please allow 72 hours for all medication refill requests.


Refill requests can be made via our email or by phone. Please provide your full name, date of birth, medication name and dosage, preferred pharmacy, and your contact information with each refill request.


Controlled substances [stimulants, sedatives]:

  • Will only be issued at a regularly scheduled appointment. 
  • No refills outside of scheduled appointments.  
  • No early refills. 
  • Lost prescriptions will not be replaced. 
  • Urine drug tests are required for prescriptions of controlled substances and frequency of testing is at the discretion of the prescriber.
  • The final decision about whether to prescribe a controlled substance is at the discretion of the prescriber.


Dismissal/Termination of Care

A patient has the right to terminate treatment for any reason. Tranquil Life Psychiatric Services reserves the right to terminate treatment at the provider's discretion.


Reason for termination/discharge from care include, but are not limited to:

  • The patient and/or patient's family member(s) threatens or engages in physical violence,  physical or emotional intimidation, verbal abuse of any kind, and/or the patient or a patient’s family member(s) carry weapons or engage in illegal acts of any kind on the premises or during treatment.
  • The patient and/or patient’s family member(s) engage in abusive phone or email correspondence.
  • The patient refuses to comply with clinic rules, refuses to comply with treatment plans/recommendations, and/or does not make a payment in a timely manner.
  • Repeated cancelations, late cancelations, and/or repeated no shows for appointments.
  • Other causes deemed reasonable by the provider.


Termination of care by either party does not relieve the patient or his/her responsible parties of any outstanding fees due.
